We offer a 10% discount to patients with out insurance.
Payment must be made with cash or check on the day of service for discount.
A service fee of $20.00 for returned checks.

Our cancellation policy is 24 hours prior to your scheduled time.
We consider your appointment broken if you do not show for the scheduled time.
Three broken appointments will result in a $50.00 charge.
We will submit to your insurance and any
co-payments will be collected on the date of service.
